What Is Body Dysmorphic Disorder?

Body Dysmorphic Disorder (BDD) is a chronic condition where individuals become preoccupied with what they perceive as defects or flaws in their physical appearance. These preoccupations can involve any body part, but common areas of concern include skin, hair, nose, and weight. People with BDD often exhibit repetitive behaviours such as mirror checking, excessive grooming, or seeking reassurance from others. The condition can lead to significant distress, social withdrawal, and impairments in work, school, or relationships. BDD is related to obsessive-compulsive disorder (OCD) and shares many features, including repetitive thoughts and behaviours.

Symptoms and Diagnosis

The primary symptom of BDD is a persistent and intrusive preoccupation with one or more perceived defects in appearance. To meet diagnostic criteria, the preoccupation must cause clinically significant distress or impairment in social, occupational, or other important areas of functioning. Additionally, the individual may engage in repetitive behaviours (e.g., mirror checking, skin picking) or mental acts (e.g., comparing with others) in response to the appearance concerns. The perceived flaws are typically slight or not observable by others. BDD is often underdiagnosed due to shame and secrecy, but awareness is growing.

Causes and Risk Factors

The exact cause of BDD is not fully understood, but it is likely a combination of genetic, biological, psychological, and environmental factors. Research suggests that abnormalities in brain structure and function, particularly in areas involved in visual processing and emotional regulation, may play a role. A history of trauma, emotional abuse, or bullying related to appearance can increase risk. Additionally, societal pressures and cultural ideals of beauty may contribute to the development of BDD. The condition often begins in adolescence, which is a time of increased self-consciousness and social comparison.

Treatment Options

Effective treatments for BDD include cognitive-behavioural therapy (CBT) and medication, such as selective serotonin reuptake inhibitors (SSRIs). CBT helps individuals challenge distorted thoughts about their appearance and reduce compulsive behaviours. Exposure and response prevention (ERP) is a key component, where patients gradually face feared situations without engaging in rituals. Medication can help reduce obsessive thoughts and depression. It is important to note that treatment should be tailored to the individual, and a combination of therapy and medication is often most effective. Early intervention can improve outcomes and prevent chronic impairment.

Coping Strategies and Support

Living with BDD can be challenging, but support is available. Self-help strategies include practising mindfulness, journaling about negative thoughts, and joining support groups. Family and friends can play a crucial role by offering non-judgemental support and encouraging professional help. It is important to avoid enabling rituals or giving excessive reassurance, as this may reinforce the disorder. If you or someone you know is struggling with BDD, reaching out to a mental health professional is a vital step. Remember, recovery is possible with the right treatment and support network.
