About the Authors

H. Gilbert Welch, MD, MPH, is a professor of medicine at the Dartmouth Institute for Health Policy and Clinical Practice. He is a leading researcher on the topic of overdiagnosis and has published extensively in top medical journals. Lisa Schwartz, MD, MS, and Steve Woloshin, MD, MS, are also professors at the Dartmouth Institute, with expertise in medical decision-making and risk communication. Together, they have authored numerous studies on the harms of excessive medical testing. Their combined experience in clinical practice and health policy gives this book an authoritative voice grounded in real-world evidence.

Key Themes and Topics

The book delves into several critical areas:

  • The Definition of Disease: How changes in diagnostic criteria have expanded the boundaries of what is considered abnormal, turning more people into patients.
  • Screening Overreach: The unintended consequences of widespread screening for conditions like breast, prostate, and thyroid cancer.
  • Financial Incentives: How the healthcare system rewards more testing and treatment, often without regard for patient outcomes.
  • Patient Perspectives: The emotional and physical toll of being diagnosed and treated for a condition that might never have caused harm.
  • Solutions: Practical suggestions for patients, doctors, and policymakers to reduce overdiagnosis while preserving the benefits of modern medicine.

Overview of the Book

In an age where medical testing is often seen as a panacea, one book dares to ask: what if more testing actually makes us sicker? Overdiagnosed: Making People Sick in the Pursuit of Health is a provocative and influential work that challenges the fundamental assumptions of modern medicine. Written by Dr. H. Gilbert Welch, Dr. Lisa Schwartz, and Dr. Steve Woloshin, this book argues that the relentless drive to detect disease early often leads to diagnoses that do not benefit patients — and may actually cause harm. The authors present a compelling case that overdiagnosis is a widespread problem in healthcare, affecting conditions from cancer to heart disease. Through vivid examples and rigorous data, they show how expanding definitions of disease, advanced screening technologies, and financial incentives have created a culture where healthy people are labelled as sick. This book is essential reading for healthcare professionals, policymakers, and anyone seeking to understand the unintended consequences of our obsession with health.

Who Should Read This Book?

This book is relevant for a wide audience:

  • Healthcare professionals seeking a deeper understanding of the hidden harms of overtesting.
  • Medical students and trainees learning about evidence-based medicine and the importance of considering downsides.
  • Policy makers and health administrators interested in reforming screening guidelines and reducing waste.
  • Patients and general readers who want to make informed decisions about their own healthcare and avoid unnecessary interventions.

What Makes This Book Stand Out

Unlike many books that celebrate medical advances, Overdiagnosed offers a critical perspective that is both necessary and timely. The authors do not simply attack medicine; they provide a balanced view that acknowledges the benefits of early detection while exposing its downsides. The book is grounded in solid research, but written in a clear, engaging style that makes complex topics accessible. It includes real patient stories that illustrate the human impact of overdiagnosis, making the data come alive. The authors also address common counterarguments, making the book a rigorous and thought-provoking read.

The Conversation It Sparks

Overdiagnosed has ignited a critical conversation in medical circles and beyond. It challenges us to rethink the balance between doing more and doing harm. The book encourages readers to question the common belief that more testing always leads to better health. By illuminating the problem of overdiagnosis, the authors advocate for a more cautious, patient-centred approach to medical care. This is a must-read for anyone who wants to understand the complexities of modern healthcare and the true meaning of the Hippocratic oath: “First, do no harm.”
